Maurizio Sarri resigned from his role as Lazio head coach, the Italian Serie A club confirmed in a statement.
Sarri, 65, left the Rome club after Lazio took five defeats in the last six games in all competitions.
His assistant Giovanni Martusciello was placed in interim charge.
Former Napoli and Chelsea boss Sarri, who won the Italian league title with Juventus in 2020, joined Lazio in 2021.
Last season, Sarri's Lazio finished the Serie A second, which was the club's best league performance since Sven-Goran Eriksson-led Lazio won the domestic title in 2000.
This season, Lazio have 40 points to be in the ninth spot in Serie A led by Inter Milan, which collected 75 points in 28 matches.
Last week, Germany's Bayern Munich beat Lazio 3-0 to eliminate the Rome club from the UEFA Champions League last 16.
